Pergamena miniata, mm. 515 x 714 il foglio, di ambito napoltano, datata 1626, testo e intitolazione in inchiostro bruno ed oro, entro elegante cornice dipinta in più colori, 6 miniature poste agli angoli ed al centro del margine superiore, raffiguranti i santi Antonio e Francesco, la Vergine col bambino, San Giovanni Evangelista, e 3 stemmi nobiliari, in calce le firme dei sottoscrittori, lacune con perdita di testo e porzioni di margini mancanti, lievi macchie, in cornice in legno moderna.
